Health Risk Assessment (HRA)
a confidential questionnaire offered online that assesses the health of your population and their readiness to make lifestyle changes developed by ActivHealth in partnership with Duke University. Each participant receives a personal report and program developed specifically for them and this strengthens an individual’s long-term commitment to achieving goals.
Online Services
To encourage behavioral change, UH&W offers a fun and interactive “path for life” along with a series of 8-10 minute classes that can be viewed at the participant’s convenience. Roam “the path” for tips and suggestions to help pave your way with fun and entertaining methods to promote healthy choices.
Coaching
Employees have access to a health coach on-line to provide support, education and resources to encourage positive behavioral changes.

Outcomes Reporting
Multiple detailed reports are provided that assess lifestyle and behavioral changes. This allows the participants to make necessary adjustments along the way which will lead to ultimate, desired results.
